LINKS Coach (Day Hab), Trivium Life Service - Cherokee

The Day Hab Coach (LINKS) is responsible for the supervision and care of individuals attending the LINKS Day Habilitation Program. The Day Hab Coach will daily, organize and prepare for the activities, outings, and personal cares of individuals under the direction of the Team Coordinator. The Day Hab Coach reports directly to the Team Coordinator.

The Day Hab Coach will have the following attributes:

• Treat persons served with the utmost respect and dignity
• Provide physical assistance to persons served as necessary in the areas of eating, dressing, personal hygiene, etc.
• Assist persons served in activities such as community outings, socialization, following directions, attention to tasks, and job readiness

• Maintain a clean, healthy, and safe environment for persons served

• Ability to push, pull, lift and/or transfer up to 50 pounds
• Good verbal and written communication skills
