Plains Area Mental Health Center is urgently hiring a Care Coordinator in our Cherokee office. This position will provide services to Integrated Health Services members. IHS is a model of healthcare delivery that utilizes a patient-centered holistic approach to coordination of care for the Medicaid eligible mental health population.

The Care Coordinator is responsible for making, tracking, and follow up on referrals. Responsibilities also include completing assessments, and the coordination of services for adults with a Serious Mental Illness and children with a Serious Emotional Disturbance.
